Introduction

The paper will provide insight into the three reflections which have been carried out, the three reflections are about common healthcare mistakes which have been observed. Gibbs's reflection is highly important as it covers all the aspects of reflections and provides an overview of the clinical incidents which are present. It will allow the healthcare workers a chance to self-introspect and act toward it.

REFLECTION 1

Description

The nurse was posted in an emergency department, and this was the first placement in the women's hospital. The patient who was admitted faced some language barriers, hence the patient was unable to understand English. I then used Google Translate, and the whole situation went badly. I forgot to report this incident to NIC, however, after reporting she asked me to contact the language bank. 

Feeling

I was feeling very sad and disheartened that I couldn't provide adequate care to the patient. Due to my nervousness and my delayed reaction the patient suffered and my health was impacted. 

Evaluation

Upon evaluating the situation it can be stated that the nurse should have complained about the situation at an earlier stage, this would not have delayed the treatment process. Poor communication can have several negative impacts decreased adherence to treatment, increased patient dissatisfaction, and along with it inefficient use of resources. There might be several reasons for poor communication among healthcare professionals, these are poor documentation, long wait times, lack of emphasis, less patient satisfaction and also providing wrong treatment. In the situation presented above, NIC should have been reported at an early stage and hence, the language bank should have been contacted at an early stage (Al Shamsi et al., 2020).

Analysis

The situation needs to be analyzed carefully, upon analysis of the situation, the problem of communication barrier is understood. It also provided an idea about the gravity of the situation hence, action needs to be taken accordingly. Some of the major factors which affect communication are concentration and attention. It affects the way of thinking and how a person solves the problem. Fatigue also affects the way communication and the impairment will get worse over time. It also decreases adherence to the treatment, patient dissatisfaction is also increased and the resources are used inefficiently (Slade & Sergent, 2020).

Action plan

As a nurse, it is necessary to have a clear understanding if the patient can understand what is being communicated. Other than this, the nurse should also respect the desire of the person to communicate. While communicating in such a situation it is important to have a good idea about the time and place so objectives can be defined clearly. The communication channel needs to be diversified, this will allow more and more people to communicate. In the action plan, all the communication plans need to be documented. In this case, cultural barriers are present, this should be addressed. When cultural barriers are taken care of then adequate treatment is provided to patients and this also increases the patient satisfaction level (Squires et al., 2018).

Conclusion

From the reflection presented above it can be concluded that language barrier is an important barrier and while present in the healthcare sector it is necessary to acknowledge it and work towards improving it. The strategies which are present to take care of this problem should be implemented effectively.

REFLECTION 2

Description

While I was working in a women's hospital ED, a 70 yrs old lady with vaginal bleeding. She requires admission to HDU for a blood transfusion Patient declined blood transfusion and admission due to a social dilemma. Escalated to Nic and contacted social support service Consultant on duty explained about the importance of blood transfusion. The social worker explained the situation and finally, she agreed to BT.

Feelings

I was feeling sad that I was unable to take adequate care of the patient. I was unable to convince the patient and this affected the well-being of the patient.

Evaluation

From the above situation it has been studied that although the treatment was necessary, however, the healthcare worker was unable to convince the patient. A lot of effort was put into convincing the patient about the importance of blood transfusion. In the case study, it has been observed that the nurse has experienced some social dilemma, due to which the patient was not agreeing to this. The healthcare worker should be such that they can easily convince the patient about the treatment. Blood transfusion is a highly important technique that needs to be followed. This technique is used because it can replace the blood which is lost, which might be due to some surgery or injury. It is also essential as it helps to prevent the body from making more blood or some of the components are made easily. Following this method will improve well-being (Lotterman & Sharma, 2022).

Analysis

Upon analyzing the situation it can be stated that there are several legal and ethical concerns which are related. In several cases the patient's body might reject blood or any blood transfusion product, hence, this needs to be taken care of adequately. In some cases, the mental health of the patient is also impacted, and this needs to be taken care of. Mental health should be maintained. The patient should be educated about the importance of blood transfusion or any treatment which is recommended. This will ensure effective care is provided to the patient and there are no loopholes in the treatment process. While providing treatment to the patient, all the ethical considerations which are present need to be taken care of. An ethical dilemma is highly important and hence steps should be adopted to control it and take care of it (Zomorrodi et al., 2022).

Conclusion

From the above incidence, it can be stated that blood transfusion is an important process. All the ethical dilemmas which are present should be taken care of exactly.

Action plan

While transfusion of blood several components need to be taken care of. The healthcare worker should be aware of the several ethical implications associated with blood transfusion. The blood banks should be contacted, for donation purposes. All the equipment which is used in blood transfusion should be sterilized adequatel (Moschidou et al., 2019).

REFLECTION 3

Description

I was placed in the emergency department, and there were several patients present. As a nurse, I was feeling overwhelmed while managing all these patients. I was managing 6 patients and was unable to do so. I escalated this to NIC, however, no help was provided. Patient care was compromised, along with patient security and safety. I faced burnout, and this affected my mental health.

Feelings

I was feeling sad and disappointed in myself because due to a lack of communication skills, the patient suffered and their safety was compromised. I should have been more careful while taking care of the patient.

Evaluation

From the situation which is present, it can be evaluated that the nurse should effectively work toward communication skills. A lack of effective communication skills will make the patient suffer. In the situation presented above the nurse should have understood the situation carefully and should have kept their calm. Although the pressure was high, if it would have managed carefully, patient safety would not have been compromised (Tiwary et al., 2019).

Analysis

Upon analysis of the situation, the importance of communication is studied and taken care of. Communication in healthcare is highly important as it will ensure that all things are taken care of. Skillful communication enables the health care to provide effective care to the patient (Murugesu et al., 2022).

Conclusion

From the situation which is described above the importance of communication is understood. When effective communication is followed then patient care will be taken care of, and burnout will be reduced.

Action plan

The action plan which is made is designed to take care of communication skills and how they can be improved in the healthcare sector. The current method of communication needs to be assessed and then changes need to be done in that section. All the communication channels need to be streamlined, this will help to bring everyone on the same page. In the healthcare sector collaboration needs to be performed between healthcare professionals (Wieke Noviyanti et al., 2021).

Conclusion

Upon conclusion, it can be stated that in each reflection the situation will be understood and steps will be taken to improve it and work towards it. A well-defined action plan is provided for every reflection, and this will help to determine if the practices are improved or not. The reflections which are stated above are some common healthcare errors and these can be corrected easily if effective steps are taken. The nurse should be able to evaluate the situation correctly and steps should be taken in that direction.
